How to Access Driver Tax Forms (1099)

1. Proceed to the eDelivery page and enter your password into the Password field.
   1. Email address is not a required field, only a password is needed to access the tax documents.
   2. The user password is the first four letters of the driver’s first name or company name and the last four digits of the driver’s TIN. Spaces and other symbols should be entered exactly as they appear.
   3. For example, if the 1099 recipient is ‘Archie Smith’ and the last four digits of their TIN is 0101, their password would be ‘arch0101’.
   4. If the recipient’s name appears as AL D with 0101 for the TIN, their password would be ‘al d0101’.
   5. If the recipient’s name appears as AL & D with 0101 for the TIN, their password would be ‘al &0101’.
   6. If the recipient’s name appears as A & D with 0101 for the TIN, their password would be ‘a & 0101’.
2. Enter the full TIN in the TIN pop-up that appears. This is required if the last 4 digits of a user’s TIN are not unique.
3. Click Submit.
4. Select the appropriate selection in the Payer section of the page. The page will redirect after a successful login.
   1. West drivers select E F Corp.
   2. Evans drivers select Evans Delivery Company, INC.
   3. Drivers that worked for Evans and West will receive a 1099 for each company and select the company they would like to view 1099 information for in the Payer section.
   4. After first log in, users will not need to select the Payer again. Password and TIN will still be required.
5. Click Opt-In & Continue in the pop-up modal that appears after Payer selection.
   1. Note: Once opted in, the user will not need to opt in again with subsequent logins.
6. Click Download PDF in the right-hand column of the line the 1099 form is on.
